CAMH places a high value on privacy, both in person and online. By accessing and using MyCAMH, you are indicating your acceptance to be bound by the terms and conditions of this Privacy Notice. If you do not accept these terms and conditions, you must not access or use MyCAMH. CAMH may revise this Privacy Notice at any time without notice. Use of MyCAMH after such changes are posted will signify your acceptance of these revised terms.
Your personal information and personal health information contained on MyCAMH will be password protected and only authorized CAMH personnel and you will have access to your information. All personal health information collected on MyCAMH will be held in the strictest confidence by CAMH in accordance with CAMH's information handling policies and procedures and in compliance with CAMH's role as a health information custodian as defined by the Personal Health Information Protection Act ("PHIPA"). You should note that records of personal health information under the custody and control of CAMH madeavailable through MyCAMH may be restricted in accordance with CAMH's records management policies and procedures. If you require access to your personal health information, you may contact our Health Records department at health.records@camh.ca or 416-535-8501, ext. 32318. CAMH's use of any personally identifying information you provide as part of the registration process is governed by the CAMH Notice and Privacy Policy (available at www.camh.ca). You are responsible for maintaining the confidentiality of the Password you use in association with your Account and are responsible for all activities that occur under your User Name and Password, including use of your UserName and Password by a third party, which is not permitted. You agree to notify CAMH immediately of any unauthorized use of your User name and/or Password or any other breach of security.
The MyCAMH portal is built using the Verto platform.
