Toronto, ON (August 14, 2026) - The Centre for Addiction and Mental Health (CAMH), in partnership with LOFT Community Services, is pleased to announce a $9.7M investment from the Ontario Government to redevelop 20,000 sq ft of the Champagne Centre, a state-of-the-art medical and wellness complex in North York, Toronto, into a permanent care environment for The Path Home program.
The investment will establish 40 transitional care beds for alternate-level-of-care (ALC) patients at the Champagne Centre, freeing up crucial hospital beds for patients in need of acute mental and physical health care. As the Champagne Centre houses a wide range of medical and wellness services, including family physicians, specialists, and medical diagnostic services, the new site ensures ALC patients are well-supported during their transition into the community.
After three to 12 months, the program's goal is to transition patients to either a long-term care facility, community housing, or one of LOFT's supportive housing programs.

"The 40-bed transitional care program will assist adults 55+ who are being supported with mental health and cognitive challenges as they wait for permanent placement in long-term care or supportive housing. This investment is part of Ontario's ongoing commitment to expanding access to specialized care."

Quick facts:
- The Path Home is a community reintegration program launched in 2021 that supports successful transitions from hospital for alternate-level-of-care (ALC) patients with complex mental health and addiction needs facing homelessness, who no longer need to be in-hospital but have nowhere to go.
- The $9.7M investment builds on the Ministry of Health’s ongoing $6.5M in funding to operate The Path Home program.
- In addition to this funding, CAMH is also investing $2.47M into the Path Home program from core hospital funds.
