This clinic offers neuropsychiatric consultation and follow-up services for adults over the age of 18 with longstanding or recently emergent neuropsychiatric disorders.

Additional Referral Details
Physician referral preferred.
Community outpatient referrals: The CAMH referral form must be completed to ensure that the referral can be processed. Please complete the Referral form and fax it to Access CAMH at (416) 979-6815.
Internal referrals: please use the ICARE Orders system within Powerchart.
Consultation by our clinic neurologist and/or neuropsychiatrist is available for both inpatient and community outpatient clients with the following conditions:
