Social Determinants of Health (SDH)

The Social Determinants of Health (SDH) Service works in the areas of housing, income, employment, and education.

The program supports the work of CAMH clinicians in these areas, develops and maintains community partnerships, works to effect system change, and leads in knowledge development and exchange.  SDH Service plays a crucial role as a leader in the psychosocial determinants of health at CAMH and oversees the SDH Champions model, a team of clinicians mobilizing a shift towards holistic health care plans that incorporate the social determinants of health at CAMH. Partnership development and capacity building in the areas of housing, income, employment, and education are critical for building better access to various services for CAMH clients in the community.
